GP Chronic Condition Management Plan (GPCCMP)

Claim up to 5 treatments through this Medicare scheme for long-term injuries.

Please note: We are not a bulk billing clinic. You will be required to pay for the appointment in full. Then we will process a medicare rebate for you. Rebates will be processed on-site with an instant response from Medicare. Rebates will be received within 24 hours, but often happen instantly.

Please bring your Medicare card & payment card to each appointment.

Workcover/TAC

We support clients with WorkCover and TAC claims, providing allied health services to assist with recovery and return to work or everyday activities.

How does it work?
Once your claim has been accepted, your treating practitioner can provide a referral or treatment plan. Our team can liaise with your insurer or case manager as needed and help coordinate your appointments.

DVA

We welcome Department of Veterans’ Affairs (DVA) clients at Lara Physio. To access physiotherapy, osteopathy & exercise physiology services through DVA, you’ll need a referral from your GP or treating medical practitioner.

We can also organise health and mobility aids for you such as walking sticks or joint supports

🟢 What does a Gold Card cover?
Gold Card holders may receive clinically necessary allied health treatment for any health condition, subject to DVA requirements.

⚪ What does a White Card cover?
White Card holders may receive DVA-funded treatment for accepted service-related conditions covered by their card.

⭐ What about a TPI Gold Card?
TPI Gold Card holders are exempt from the standard treatment cycle for physiotherapy and exercise physiology, meaning treatment can continue under an annual or indefinite referral where clinically necessary.

📋 Do I need a referral?
Yes. Most DVA-funded allied health treatment requires a valid referral, usually from your GP. Standard referrals cover up to 12 sessions or 12 months, whichever comes first.

NDIS

We welcome NDIS participants at Lara Physio and provide physiotherapy, osteopathy, exercise physiology, and other allied health services to support your goals and independence.

We work with self-managed and plan-managed NDIS participants. Your NDIS plan or support coordinator can help determine whether our services are included in your plan.
